If the columns 3-82 are filled up before a child leaves School, his name is to be re-entered and a new register number given as for a new pupil, but the former register number should be written underneath, thus 63/5.

MEMORANDUM FOR TEACHERS OF STATE SCHOOLS.—
QUARTERLY RETURNS OF ATTENDANCE.
State School Teachers are requested to carry out carefully the following instructions relative to the Quarterly Returns of attendance furnished to the Department:—
1. The Return should shew in the heading the period for which it is compiled.
2. The names of the pupils should be entered alphabetically (surnames first) in the order of the classes.
3. The Christian names and surnames of parents or guardians, with their addresses, should be inserted.
4. When a child has obtained a certificate that he has been educated up to the standard, an entry to that effect should be made in the explanation column.
5. The reasons given by children for their irregular attendance should also be entered in the explanation column of the Return, in which moreover the dates of the last attendance of old scholars, and of the first attendance of new scholars, should be given.
6. The Returns should be transmitted to the Office not later than the 15th day of the month succeeding the quarter to which they refer.

The List of Children who attended less than thirty days during the quarter should not include the names of any who—

Above 9 and under 12, within 2½ miles;
Above 12, within 3 miles);
or,
(4) Have obtained their Certificates.

“The Education Act 1889” having rendered an amendment of the Quarterly Return Form necessary, State School teachers are requested to carry out carefully the following instructions relative to the compilation of that Return:—
1. The Return should be compiled in accordance with the headings. The cover should be properly filled in, and the number of the school given upon every sheet.
2. The Return should be arranged in the order of classes, the names of the pupils in each class being entered alphabetically.
3. During the last week in each Quarter the correct address of the parent of each child who attended the school should be ascertained, and care should be taken that it is rightly stated in the Return.
4. In the case of a child who has attended less than the required number of days, the explanation column should show:—
(a) The reasons (of which a record should be kept) given for his insufficient attendance:
(b) Whether he has obtained a certificate of exemption:
(c) When he is thirteen years of age, and in the fourth or a higher class, whether, since he entered the fourth class, he has or has not attended school according to the law in force at the time for four Quarters of a Year.
(d) Whether he has entered or left the school during the Quarter, in which case the date of entry or departure, and, if possible, the name of the school from which he has come or to which he has gone, should be given:
(e) When he is returned as residing beyond the prescribed distance, whether his residence is nearer to another school.
5. The Return should be transmitted to this office not later than the 7th day of the month succeeding the Quarter to which it relates.
